Provider referrals are not necessary for individuals seeking treatment. However, if you are a healthcare provider and would like to assist your patient in their addiction recovery and/or pain management process, please fax a referral to 541-779-2081 to the attention of Reba Smith, Program Manager. Please include a contact number for the patient, and our admissions coordinator will contact the patient directly. We welcome patients to contact us directly. Call 541-779-1282, visit our Walk-In Clinic which opens at 8:30 am, Monday through Friday.

Healthcare providers are welcome to contact us at any time for more information regarding our services, to speak with an admissions coordinator, or request on-site education. Please contact Sarah Walker, provider relations director, at 541-779-1282 x20204 for more information.

Insurance Information

Addictions Recovery Center accepts most major insurance and OHP plans. For those who are uninsured, we offer a sliding fee scale and payment options. Please call 541-779-1282 x115 regarding insurance, payment, and admissions information.

Referring Patients to the Pain Management Program

Our Pain Management Program offers comprehensive screenings, assessments, education, counseling and case management. All staff are certified alcohol and drug counselors with extensive knowledge of the impact of chronic pain on daily life. We work individually with each patient to provide the best combination of services to facilitate the safe management of pain and a return to a healthy lifestyle.
